You have the ability to create fillable PDF forms using Adobe Acrobat Pro using existing digital documents (PDFs, Word documents etc.), or you may scan a paper document using any copy machine on campus. Adobe products are available to all faculty and students.

PC users, please submit a helpdesk request  if you would like a copy of Adobe Acrobat Pro installed.
Mac users should use our Self Service application to install Adobe Acrobat Pro.

Important considerations:

Please consider the purpose of the fillable PDF form you are about to create. Fillable PDF forms mimic paper forms. If you distribute the form to twenty people, you will receive a separate PDF document from each of the respondents. If your intention is to collect a lot of data from several people (as in a survey), a service such as SurveyMonkey or Google Forms might be more appropriate.

Fillable PDF forms do not natively work on mobile devices (phones, iPads etc.). The recipient of a fillable PDF form would need to install the mobile version of Adobe Acrobat Reader (free) in order to respond from a mobile device.

Step 1:

Launch Adobe Acrobat Pro DC (this program is available to all faculty and students at GPS).

Step 2:

Within Acrobat, click on the Tools tab.


Step 3:

In the right column, click on Prepare Form.


Step 4:

Click on the Select a file button, the click the blue Start button.


Step 5:

Acrobat will analyze your document and will try to add form fields automatically. Note the toolbar (below) containing form fields you may want to insert into your form.


Step 6: 

Once you have created your form by assigning all your form fields, click on the Distribute… button in the right-hand column.


Step 7:

You will be asked to save the form. Click the Save button.


Step 8:

Name your form and find a place where you would like to save it.


Step 9:

Make sure Email is selected and click the Continue button when asked how you would like to distribute the form.


Step 10:

Next you want to make sure that “Save a local copy and manually send it later” is selected, then click the Next button. The program will remember where you save the form in step 8.


Step 11:

On the next pop-up, click the Finish button.


Step 12:

Click the red X to close the next window. There are features in Adobe Acrobat Pro that will allow you to collect responses that is currently not available to us.


You should now have a document saved to your computer that you can email to your recipients as an attachment. The fillable PDF document that you just created mimics the use of a paper form that you would distribute to a group of people. For surveys or collection of data that needs to be analyzed we highly recommend the use of Google Forms or Survey Monkey.
