You have the ability to create fillable PDF forms using Adobe Acrobat Pro using existing digital documents (PDFs, Word documents etc.), or you may scan a paper document using any copy machine on campus. Adobe products are available to all faculty and students.

PC users, please submit a helpdesk request  if you would like a copy of Adobe Acrobat Pro installed.
Mac users should use our Self Service application to install Adobe Acrobat Pro.

Important considerations:

Please consider the purpose of the fillable PDF form you are about to create. Fillable PDF forms mimic paper forms. If you distribute the form to twenty people, you will receive a separate PDF document from each of the respondents. If your intention is to collect a lot of data from several people (as in a survey), a service such as SurveyMonkey or Google Forms might be more appropriate.

Fillable PDF forms do not natively work on mobile devices (phones, iPads etc.). The recipient of a fillable PDF form would need to install the mobile version of Adobe Acrobat Reader (free) in order to respond from a mobile device.

Step 1:

Launch Adobe Acrobat Pro DC (this program is available to all faculty and students at GPS).

Step 2:

Within Acrobat, click on the Tools tab.

In the right column, click on Prepare Form.

Click on the Select a file button, the click the blue Start button.

Acrobat will analyze your document and will try to add form fields automatically. Note the toolbar (below) containing form fields you may want to insert into your form.

Step 6: 

Once you have created your form by assigning all your form fields, we will add a submit button that the user can click and email the filled out PDF back to you. Find and click on the button form field  11 and insert it by clicking in an area on your form.

Step 7: 

Name the button “Submit” and click the All Properties link.

Under the Options tab, type Submit beside Label.

Under the Actions tab, click Add...

(1) Under Enter a URL for this link: enter mailto:myemail@gps.edu replacing myemail@gps.edu with the email address where you want the forms to be sent when a recipient of the form clicks on the Submit button. (1) Under Export Format, make sure PDF The complete document is selected. Click OK, then Close.

Step 11: 

Save the document by clicking on the File menu, then Save as… 

Step 12:

Send the document to the intended recipients.
